Membership of the Royal College of Emergency Medicine (MRCEM)

MRCEM is the Royal College of Emergency Medicine's membership-by-examination pathway in Emergency Medicine. To achieve MRCEM and be eligible for College Membership, candidates must pass the components in sequence: MRCEM Primary, MRCEM SBA, then MRCEM OSCE.

RCEM lists MRCEM OSCE delivery in London, Kuala Lumpur, Chennai, and Hyderabad. These are delivery locations for the same qualification, not separate national versions. RCEM regulations state that internationally delivered examinations are written, developed, and standard set in the same way as examinations delivered in the United Kingdom.

The current RCEM examination page uses the name MRCEM SBA. The linked clinical blueprint pack retains the earlier MRCEM Intermediate SBA heading, so candidates may encounter both labels in official material.

From January 2026, RCEM theory examinations have been delivered through Surpass Assessment. The College prefers test-center delivery, while exceptional remote delivery may be considered when candidates cannot travel and meet the specified technical requirements. Older component packs may still refer to Pearson VUE, so current delivery and booking instructions should come from the current RCEM theory, application, and calendar pages.

MRCEM Primary

The current RCEM examination page points candidates to the Basic Sciences Curriculum and the linked Primary information pack. The published 180-question blueprint is:

For the October 27, 2026 MRCEM Primary examination, RCEM states that the current Basic Sciences Syllabus remains applicable. From the April 21, 2027 MRCEM Primary examination onward, the updated Basic Sciences Syllabus becomes the official reference document. RCEM says the updated syllabus will build on anatomy, physiology, pharmacology, microbiology, pathology, and evidence-based medicine.

The linked official blueprint pack uses the former MRCEM Intermediate SBA heading, while the current examination page uses MRCEM SBA. Its clinical blueprint is:

MRCEM OSCE

The OSCE pack allocates stations across Specialty Learning Outcomes:

The OSCE information pack describes assessment of the evaluation and immediate management of common adult and pediatric emergency conditions, together with clinical and communication skills.

Candidates must meet the relevant criteria at the point of application, not only on the examination date.

Additional requirements and rules to check before applying:

• For all exams, candidates must be registered with the GMC or with the statutory medical regulator in the country where they practice, without limitation on practice. Registration investigations, suspensions, limitations, or removals must be declared.
• Candidates have 7 years to complete the MRCEM components from passing MRCEM Primary. Case-by-case extensions may be considered in specified circumstances.
• The standard maximum is 6 attempts at each MRCEM component. After 6 failed attempts, a candidate may request 1 additional attempt through the College's process; approval is case by case.
• Officially enrolled DRE-EM trainees may be granted an exemption from MRCEM Primary if they have achieved MRCS. RCEM states that there are no other exemptions or recognized equivalents from other medical colleges or institutions.
• Applications are submitted online during the published application window. Candidates should prepare medical registration evidence, internship or Foundation-equivalent evidence, and English translations where required.
• All RCEM exams are conducted in English. Candidates applying for MRCEM OSCE must have achieved at least IELTS Level 7, OET, or an equivalent standard, with evidence potentially required at application.
• Provisional applications to a subsequent MRCEM component while a previous result is pending are not accepted.

1. Plan around the sequence

Prepare for MRCEM Primary, then MRCEM SBA, then MRCEM OSCE. Treat each component as a distinct assessment with its own blueprint, timing, and practice method. Confirm that prerequisites and application evidence are complete before booking.

2. Build the Primary foundation by blueprint size

Start with anatomy and physiology, which together account for 120 of the 180 published questions. Then organize pharmacology by emergency-relevant systems and treatments, followed by microbiology, pathology and hematology, and evidence-based medicine. Use active recall, spaced repetition, diagrams, calculations, and timed single-best-answer practice rather than passive reading alone.

For a Primary sitting on October 27, 2026, use the current Basic Sciences Syllabus. For the April 21, 2027 sitting or later, use the updated syllabus once it becomes the official reference for that examination date.

3. Study MRCEM SBA by SLO and clinical task

The largest SBA areas are SLO1 complex stable patients, SLO3 resuscitation and organ failure, and SLO4 injured patients. Use a consistent emergency-department reasoning structure: identify the immediate problem, select focused assessment and investigations, choose initial management, reassess, escalate, and plan disposition. Give dedicated time to pediatric emergencies, procedural skills, safeguarding, law, information governance, evidence, and guidelines because they are explicit blueprint categories.

Rehearse the actual timing: two 90-question papers, each completed in 2 hours, with a 1-hour break. Review why each distractor is less appropriate, not only why the selected answer is correct.

4. Train for OSCE performance, not only written knowledge

Practice 8-minute stations with 1-minute reading periods. Rotate through stable patients, answer-questions stations, resuscitation, injured patients, pediatric emergencies, procedures, complex or challenging situations, and supervise-and-teach stations.

Use a consistent opening routine: identify the task, clarify immediate risk, structure the encounter, communicate your reasoning, and finish with a clear plan. For procedural and resuscitation stations, rehearse verbalization, equipment checks, escalation, teamwork, and reassessment. Do not treat written SBA, SAQ, or structured oral-exam practice as a substitute for timed OSCE station work.

5. Use an administrative checklist

Confirm the current venue and booking instructions through RCEM, check identification requirements, review any reasonable-adjustment documentation, and allow time for travel or visa arrangements where relevant. Use the official calendar and application pages rather than older information-pack instructions for current dates, fees, application windows, and delivery arrangements.

Primary syllabus transition

The syllabus boundary is date-specific. The current Basic Sciences Syllabus applies to the October 27, 2026 MRCEM Primary examination. The updated Basic Sciences Syllabus becomes the official reference from the April 21, 2027 MRCEM Primary examination onward. Candidates should not switch to the future syllabus for the October 2026 sitting solely because the change has been announced.

Current delivery versus older documents

The 2021 component information packs remain useful for the published blueprints, but they contain historical delivery references. RCEM's current theory FAQ states that theory examinations are delivered through Surpass Assessment, and the current application page describes the Surpass test-center network. Follow current RCEM booking instructions when an older pack conflicts with a current webpage.

UK and international candidates

The same MRCEM components are delivered across the listed locations. International delivery is not a separate national version: RCEM states that international examinations are written, developed, and standard set in the same way as UK examinations. Candidates outside the United Kingdom must still satisfy the component-specific qualification, registration, experience, language, documentation, and application requirements.

Candidates traveling for an OSCE are responsible for the visa and entry requirements of the destination country. RCEM regulations state that allocated OSCE candidates receive a visa support letter after payment, but receiving a letter does not remove the candidate's responsibility to obtain the required permission to travel.

OSCE allocation

OSCE places are limited. Approval of an application does not itself confirm an OSCE place; allocation is handled separately under the College's current oversubscription arrangements. Check the relevant calendar entry and application guidance for the diet and location being targeted.
